Staying Safe at your Longevity Appointment:

For your upcoming appointment!

We are so happy to have you back in the office!!!

Please abide by the following rules to stay safe and healthy: 

1. When arriving to your appointment do not enter until your therapist comes to get you. They will open the door for you and take your temperature.  

2. Fill out COVID-19 waiver that will be outside the front door 

3. Limit personal belongings to the clothes you are wearing with no additional purses/jackets/etc.  

4. Face masks are required while in the office and while laying face up on the table.  Please enter the building with one or one will be provided 

5. Immediately entering the building proceed to the restroom to wash your hands for a minimum of 20 seconds

6. A card on file needs to be provided prior to your appointment time. No cash or credit card handling will be happening at the desk 

7. Hang clothes on the back of the door
